Please note some insurance companies have relaxed signature requirements and we may be able to email you and not require you to actually attend our offices.

- Are you currently under self-isolation?
- Are you currently under quarantine?
- Have your travelled in the past 14 days?
- Do you or any person in your family have a cough?
- Do you or any other person in your family have a fever?
- Are you or any other person in your family experiencing shortness of breath?
