Please note some insurance companies have relaxed signature requirements and we may be able to email you and not require you to actually attend our offices.

Please do not attend any of our offices if you can answer “Yes” to any of the following:

Are you currently under self-isolation?
Are you currently under quarantine?
Have your travelled in the past 14 days?
Do you or any person in your family have a cough?
Do you or any other person in your family have a fever?
Are you or any other person in your family experiencing shortness of breath?
